•   Data of persons in charge of sterilization and disinfection operations (educational qualifications) (only for companies practicing sterilization and disinfection activity)

•   Safety checks for used chemicals (factories, if any, building cleaning companies, companies practicing sterilization and disinfection activity)

•  Attach the previous building cleaning permit (building cleaning companies only)

•   Previous environmental permit in case of license renewal or license data modification
